Before you come to our practice for your first appointment, please take a minute to print and fill out the patient information forms.

Accepted forms of payment are cash, checks, credit cards.

A credit or debit card is required for all NEW appointments. Cards are to guarantee the appointment, and would only be charged in the event of a no-show or last minute cancellation or reschedule.
